For those scheduling through the Bell County system all of your "forms" are completed electronically so you no longer need to complete paper forms. For those who still need the paper forms download a vaccination packet here and your second dose you can download this form.
For the second dose, please be aware that you should not get your second dose too early, and the CDC does recommend not waiting longer than 6 weeks (42 days) but if you go beyond 42 days you do not need to restart the series.

ELIGIBILITY FOR THE VACCINE IS CURRENTLY LIMITED TO TIER 1 & 2 AND THOSE WITH HEALTH CONDITIONS OR OVER THE AGE OF 65. ONLY THOSE FITTING APPROPRIATE CRITERIA SHOULD REGISTER AT THIS TIME.

Appointment availability is based on vaccine availability. If you do not see an available appointment keep an eye on the calendar for additional openings as more vaccine is delivered. Vaccine appointments are planned to open once a week on Tuesday's beginning February 9th as long as vaccine delivery is confirmed.
