Health Card Classes

Bell County Public Health now offers a free 2-week loan program for our Accredited Retail Food Handler Program DSHS License #95. There are 2 different formats the program is offered in: DVD or Flash drive. There is a $5/day late fee if not returned by the due date annotated at check out by Bell County Public Health. Once you have watched the program, submit the form via email, fax, or walk in. We will get the card(s) ready for you and let you know when to come for payment and pickup. You can pay over the phone by credit card, additional fees apply. 

Two-year card     $20.00

(Now accepting cash, debit or credit cards – there is a fee charged to your credit card based on the amount you charge – minimum fee $3.00
